Max's parents leave, leaving only Kenneth there. Kenneth smiles deviously, Max is scared. "You're coming home with us," he says, "But we've got to get you changed. Can't have you going home like you are."
Kenneth opens the bag he had been carrying around and reaches in. Max can't see what it is yet. Kenneth pulls it out and puts it on the bed. Max can see that it's not really a costume, rather a teddy bear that has been hollowed out. He is confused but he quickly realises. Kenneth is going to put that teddy bear on him.
Kenneth wastes no time. He pulls off Max's hospital clothes. Max tries to stop him but having stumps for limbs makes it nigh impossible. Kenneth touches Max's penis. It twitches and Ken laughs, "You're so horny and vulnerable now. This is going to be fun." Max thinks he knows what's going to happen, but it doesn't. Instead, Kenneth puts a 'half-diaper' on Max. The front is open but the back isn't. Kenneth smiles at his helpless, diapered brother and ruffles his hair.
Kenneth opens up the teddy bear and drags it up Max's limbless body. Max has some control over the bear's limbs but not too much, they're still fairly floppy. Kenneth zips up the bear and locks the zip. He backs off and looks at Max. He smiles and takes a picture on his phone. Max is angry and depressed. His brother has taken advantage of his situation. Kenneth shows Max the picture. The only thing that shows he's human is his head. Everything else below looks 100% brown teddy bear. It's actually a little terrifying.
Kenneth picks Max up and puts him over his shoulder, and begins to pat him. "It's alright. We're all going to look after you," he says cooing. He puts Max against the wall on the floor and begins collecting Max's stuff, including the bear. He then picks Max up and walks out of the room.
