"I want a vagina," you say.
She looks at you, slightly surprised, but not shocked. "That's not an unreasonable request," she says. "How long have you felt this way?"
"A long time," you say.
She nods. "And you're sure about this? Have you tried living as a woman?"
"Doctor, I’m not a woman," you say. "I'm a man and I'll always be a man, but I don't want a penis and balls. I want to be a man with a vagina."
She smiles. "Okay, then. Let's talk about the procedure."
She explains the procedure to you in great detail, and you listen intently, excited at the prospect of finally getting what you've always wanted. First, they’ll remove your testicles, and then they'll use tissue from your penis to create a clitoris and labia minora. Finally, they'll create a vaginal canal with skin from the penis and discard the rest.
"And then," she says, "you'll have a functioning vagina."
"It'll be real?" you ask.
"Of course," she says, sounding slightly offended. "Everything we do here is real. If you don't believe me, ask any of my patients."
"I'm sorry," you say, blushing. "It's just... I've dreamed of this for so long."
Just two weeks later, you are ready. You're laying in the operating room, wearing nothing but a hospital gown, your legs up in stirrups. The room is cold, and you're shivering with excitement. The doctors and nurses are busy preparing for the operation, and you can't wait for it to begin.
Finally, the anesthesiologist approaches you, smiling reassuringly. "We're going to put you under now," he says.
You nod, and he places the mask over your face. You take a deep breath, and the world starts to go fuzzy around the edges. Just before you lose consciousness, you hear the doctors saying something about removing your testicles, and the last thing you see is the scalpel glinting in the bright light of the operating room.
When you wake up, everything feels different. Your body is sore and swollen, and the first thing you notice is the lack of pressure between your legs. You reach down and feel a bandage where your penis and balls used to be, and a smile spreads across your face.
